I certify that all of the information included in this application is accurate to the best of my knowledge.  I understand that knowingly withholding information or providing false information is grounds in notifying employer, and any misrepresentation of facts on this application can be considered cause for discontinuance of service by Total Tours 360.

I agree that any information that I may obtain during the application process is confidential. I can not share information about Total Tours 360 policies, procedures, commission structure, photography techniques or other business and technical information with any third party. I am not authorized to reproduce or copy any documents given to me by Total Tours 360 unless I have written permission.

I understand that Total Tours 360 may, at its own discretion, conduct a full background check. This background check may include criminal history, credit check, driving record and social security trace and verification.  Further, I authorize Total Tours 360 to contact and solicit appropriate past or present employers and educational institutions, governmental occupational licensing or registration entities, business or personal references, and any other source required verifying information that I have voluntarily supplied; and that it is understand that I may request a complete and accurate disclosure of the nature and scope of the background verification, to the extent such investigation includes information bearing on my character, general reputation, personal characteristic or mode of living. I authorize without reservation, any party or agency contacted to furnish the above mentioned information and release all parties involved from liability and responsibility for doing so. This authorization and consent shall be valid in original, fax, or copy form.
